We recognize that when you join an online support group (OSG) via the Internet you want to be sure that everyone in these OSGs is either a teen with cancer or a parent/guardian of a teen with cancer. Therefore, below are the registration forms that you are required to complete before starting Group Loop’s OSGs. Please complete only the forms that apply to you.

Teen Diagnosis Confirmation Form - This form makes sure that you are eligible to participate in the OSG

Parent Permission Form - If you are under 18, we need this form signed by your parent/guardian and returned to us.

Parent Confirmation Form - This form confirms that you are a parent/guardian of a teen with cancer and gives us permission to contact your teen’s doctor.

The Wellness Community and Childrens Hospital Los Angeles (CHLA) have joined together on a research study to look at whether online support groups help teens with cancer better cope with the stress of the illness.  

• If you are a teen under 18 years old, you need to complete the following forms:
  • (1) Assent Form
  • (2) Parental Consent Form
• If you are a teen 18 and over, you will need to complete the following form:
  • (1) Adult Consent Form
